Table-record documentation

Dear folks,

I've put documentation for the EPICS 3.13 table record in the EPICS
software distribution Web pages, at
http://www.aps.anl.gov/xfd/SoftDist/swBCDA/table/tableRecord.html
The documentation isn't completely finished, and it describes an
unreleased version (4.0) of the record, but nearly all of it applies
to earlier versions as well.

By the way, all of XFD's Web pages have moved from
http://www.aps.anl.gov/xfd/WWW/xfd/...
to
http://www.aps.anl.gov/xfd/...
